New Clients

Within our multidisciplinary setting, we believe in thoroughly considering each child’s unique and individual needs when developing an appropriate treatment plan. Therefore, we have designed a comprehensive referral process to effectively perform this service.

Step 1

Referral: All referrals and inquires about our services are handled by Molly Buist, OTR, who can be reached or phone at (616) 667-9551, ext 109.

You can also fill out our patient information forms prior to your initial intake appointment:

  1. Client Information Form and Financial Policy (.pdf)
  2. Client History Form (.pdf)
  3. Client Sensory Questionnaire for Occupational Therapy (.pdf)

Step 2

Initial Intake: An initial intake appointment will be scheduled with our intake coordinator. At that time, you will be able to tour the facility, ask questions and discuss your concerns.  We will review client history including any prior evaluations and/or treatments.  We will help you decide whether an evaluation is appropriate and which types of assessments will best meet your needs.

Step 3

Evaluation: Following the initial intake, the client may be recommended for one or more of the following assessments based on their individual needs:

Further in depth information regarding these assessments will be provided upon recommendation/request.

Step 4

Therapy: We offer both individual and group services.  Frequency and duration of treatment is determined based on the client’s individual needs.  Parent participation is considered an essential part of the therapy process. Our therapists provide ample parent consultation time as part of each treatment session and work with parents and educators to incorporate therapeutic interventions into all aspects of the client’s life.
